Journey to Wellness Supports Coordination Services Journey to Wellness locates, coordinates and monitors services for individuals with intellectual and Question: What Is Support Coordination?

Answer: "Support coordination is the service of advocating, identifying, developing, coordinating and accessing supports and services on behalf of a recipient, or assisting the recipient or family to access supports and services on their own. These services may be provided through waiver and Medicaid State Plan services, as well as needed medical, social, educational, other appropriate services, and community resources regardless of the funding source through which access is gained." I am the single mother of a 21 years old daughter who was diagnosed with Cerebral Palsy. Caring for my daughter has been very rewarding, but as any caregiver knows, it has its challenges. I spent countless nights researching supports that could make things a little easier for my daughter and I. I decided that I wanted to make a difference in the lives of individuals and their family's with similar challenges.

❤️ 20 Years Later, I Still Have This Necklace… ❤️
Sometimes the smallest gestures leave the biggest impressions on our hearts. 🥹
About 20 years ago, when Cortezia was still very young and I was still learning how to navigate life as a mother of a child with special needs, her father and I decided to take her to a skating rink.
Now, obviously, my baby couldn't skate, but that wasn't going to stop us from letting her experience the joy of being there. So we put some skates on her little feet, and we pushed her around the rink in her wheelchair. ❤️
But I remember being a nervous wreck that day. Not because of the skating, but because I didn't know how the other children would react to her. Would they stare? Would they understand? Would they treat her differently?
And then something happened that I will never forget.
A little girl, probably only a couple of years older than Cortezia, walked up to us and asked if she could give my baby this necklace.
Y'all, the emotions I felt in that moment. 🥹❤️
Here I was, so worried about how the world would receive my daughter, and this little girl, with the purest heart, showed us kindness.
She didn't know our story. She didn't know my fears. And she certainly didn't know that something so simple would mean so much to a mother who was still trying to figure it all out.
Twenty years later, we STILL have that necklace.
Through moving, packing, unpacking, and all the changes life has brought us, I have made sure that necklace stayed with us. Today, it hangs in Cortezia's room.
Not because of what it's worth, but because of what it represents.
Kindness. Acceptance. Compassion. And a beautiful reminder that sometimes the world is a lot more loving than we fear it will be. ❤️
I don't know where that little girl is today. She's a grown woman now, probably with a family of her own. But I wish I could tell her that something she did as a child has stayed in my heart for two decades.
And if she ever happens to see this post, I hope she knows that her little act of kindness meant more than she could ever imagine.
Be kind. You never know what someone is carrying in their heart or how your smallest gesture might become a memory they treasure for a lifetime. ❤️

💜 THE HARDEST PART OF BEING A SPECIAL NEEDS MOM…
In my opinion, one of the hardest parts of being a mother to a child with special needs isn't necessarily the day-to-day caregiving, the appointments, or even the challenges we face along the way.
It's thinking about the future.
It's asking yourself those questions that no parent ever wants to think about.
💜 What happens to my child if something happens to me?
💜 Who will love, protect, and advocate for my child the way I do?
💜 Will my child receive the same level of care, compassion, and understanding?
💜 What will my child's life look like when I'm no longer here?
And on the other side of that conversation is another painful reality that many of us don't even want to imagine: What would our lives look like if our child were no longer here?
These are uncomfortable conversations. They're emotional. They're frightening. And quite frankly, they're conversations many of us would rather avoid.
But avoiding the conversation doesn't eliminate the possibility of having to face these realities.
As a mother of a daughter with cerebral palsy, I understand these fears personally. I understand the sleepless nights, the uncertainty, and the overwhelming responsibility of wanting to make sure your child is protected, supported, and loved, no matter what tomorrow brings.
That's why I believe planning for the future is one of the greatest acts of love we can give our children.
At Journey to Wellness Supports Coordination Services, LLC, we understand the importance of helping individuals and families prepare for life's transitions.
Whether it's exploring future living arrangements, identifying support systems, connecting with community resources, or discussing long-term care needs, our agency can help families navigate those difficult conversations and begin developing a plan.
We may not be able to predict the future, but we can take steps today to better prepare for it.
💜 Because planning for tomorrow doesn't mean we've given up on today. It means we love our children enough to prepare for a future we may not always be able to control.
Let's start the conversation.
To my fellow parents and caregivers: Have you started planning for your loved one's future, or is this a conversation you've been putting off?
