⚠️ OSHA proposed $172,324 in penalties against a Florida roofing company after one worker suffered fatal injuries and another was seriously injured in a fall from a two-story home.

According to OSHA, the two workers were installing a tarp in September 2025 when they slipped from the roof and fell more than 20 feet into an empty swimming pool. Investigators found that the employer allowed them to work without personal fall protection. OSHA also cited failures involving fall-hazard training and a hazard communication program.

OSHA published its findings in April 2026. We are sharing this story for educational purposes and as a reminder that roofing work can become deadly when crews lack proper fall protection, training, and jobsite planning.

🚧 Weekly Safety Talk #4: OSHA Anchor Point Requirements 🚧

What Counts as an OSHA-Compliant Anchor Point?

Under 29 CFR 1926.502(d)(15), an anchorage must either support at least 5,000 pounds per worker attached or be designed, installed, and used as part of a complete personal fall arrest system that maintains a safety factor of at least two under the supervision of a Qualified Person.

That second compliance path is the part many crews never hear about.

An OSHA-compliant anchorage must also be independent of anything supporting or suspending a platform. Guardrails and hoists cannot be used as fall arrest anchor points, no matter how sturdy they may appear.

But what is the difference between a Competent Person and a Qualified Person? Can a roof truss serve as an anchor? How many workers can connect to one anchorage? What changes when using fall restraint instead of fall arrest? And how often should anchor points be inspected or recertified?
