𝗔𝗜 𝗠𝗲𝗲𝘁𝘀 𝗣𝗿𝗲𝗰𝗶𝘀𝗶𝗼𝗻: 𝗧𝗵𝗲 𝗙𝘂𝘁𝘂𝗿𝗲 𝗼𝗳 𝗜𝗖𝗟 𝗦𝗶𝘇𝗶𝗻𝗴
One of the most exciting advances in refractive surgery today is the integration of artificial intelligence into ICL planning.
UBM AI + ICL Guru by Sonomed Escalon is redefining how surgeons determine the optimal ICL size, helping improve predictability, efficiency, and patient outcomes.
Using high-resolution UBM, the platform automatically analyzes more than 25 anatomical parameters and applies a deep-learning AI model to predict postoperative vault, angle configuration, rotational stability, and other critical metrics. Rather than relying on indirect measurements, the system evaluates the actual anatomy where the lens will sit, providing highly individualized recommendations.
The UBM AI workflow further enhances accuracy through intelligent eye tracking, automated image alignment, quality scoring, and image selection, ensuring that only optimal scans are used for analysis.
Recent multicenter clinical data involving nearly 350 EVO ICL patients demonstrated remarkable agreement between predicted and actual postoperative vault measurements, with most cases showing less than a 100-micron difference and no lens exchanges due to sizing errors.
As AI continues to evolve, technologies like UBM AI + ICL Guru are helping move refractive surgery toward a new standard of personalized, data-driven care.

Selecting the right tool is important to get a job done with quality and efficiency. When it comes to medical imaging, tool selection is paramount for accuracy, as well.
For the job of sizing ICL lenses using ICLguru, Sonomed Escalon provides the optimized 35 MHz transducer that shows excellent resolution of the entire anterior chamber, including most importantly the posterior surface of the lens so that proper image alignment can be quickly assured and accurate parameters fed into the ICLguru AI algorithm.
Combined with the Sonomed Escalon UBM AI, well-aligned UBM imaging can be quickly and confidently obtained with the 35 MHz transducer.
Conversely, Sonomed Escalon also offers a 50 MHz UBM transducer, which provides stunning imaging of the angle and iris plane, but which does not provide the scan depth necessary to reliably image the posterior lens surface.
📌 This is due to the physics of energy waves in which lower frequencies propagate greater distances (think of whales using ultra low-frequency sound waves that can travel hundreds of miles).
✔️ 35 MHz = deeper pe*******on for ICL sizing
✔️ 50 MHz = enhanced detail for angle and iris imaging
The 35 and 50 MHz transducers are easily interchangeable on Sonomed Escalon systems so that the right frequency can be matched for the job at hand.
For ICLguru, the 35 MHz transducer frequency is clearly the right tool for the job.

ICLguru assesses the full anatomy of a patient’s anterior segment, including where a lens will actually sit, to determine optimal ICL sizing based on standardized imaging provided by Sonomed Escalon UBM AI, which stands in contrast to other sizing methods reliant upon nomograms and measurements only from the iris forward:

◼️ No proxy measures to estimate sizing – Sonomed Escalon UBM AI images the entire anterior segment including the ciliary body and other structures behind the iris plane needed to accurately predict ICL sizing

◼️ No assumptions of simplified linear relationships between a limited number of variables – ICLguru is a complex deep-learning model that utilizes over 20 biometric parameters

◼️ No more rules of thumb – ICLguru provides individualized vault safety bands based on unique patient anatomy

◼️ No mere recommendation of a single lens size – ICLguru provides actual predictive data for central vault, peripheral vault, angle, and rotational stability for each ICL size

◼️ No subjectivity of measurements by using different devices – Sonomed Escalon UBM AI provides validated auto-measurements directly to ICLguru, thereby ensuring repeatability and saving clinic workflow time

◼️ No surprises and outliers requiring explant or exchange – surgeon after surgeon recount their experience in driving their need for exchange to zero once adopting Sonomed Escalon UBM AI + ICLguru

◼️ No more worrying about ICL sizing – Sonomed Escalon UBM AI + ICLguru provides confidence to surgeons they will select the right lens size, the first time, every time
