Sports Podiatry involves the diagnosis and the treatment of foot, ankle, leg and knee injuries utilising a variety of different methods ranging from insoles, orthotics, footwear and exercises specific to each individual sport. We can also refer for ultrasounds and x-rays and work alongside the sports medicine team of physiotherapists, sports physicians, and orthopaedic specialists. Tendons are tough, fibrous tissues that connect muscle to bone, enabling movement, stability, and balance in the foot. The primary tendons of the foot and ankle, which dictate its major motions, originate from muscles in the lower leg and route to various insertion points.
The major foot tendons and their primary functions include:
• Achilles Tendon: The largest and strongest tendon in the human body, it connects the calf muscles (gastrocnemius and soleus) to the heel bone (calcaneus). It allows for plantar flexion, which is pointing the foot and toes downward to push off the ground when walking or running.
• Posterior Tibial Tendon: Originates deep in the calf and runs down the inside of the ankle. It inserts into the bones of the midfoot and is primarily responsible for inverting the foot (turning it inward) and supporting the foot's arch.
• Anterior Tibial Tendon: Runs down the front of the shin to the midfoot, allowing for dorsiflexion (pulling the foot and toes upward toward the shin).
• Peroneal Tendons (Longus and Brevis): Run along the outside of the ankle and foot. They pull the foot outward (eversion) and help stabilize the outer ankle and arch.
• Extensor Tendons: Located on the top surface of the foot, these tendons connect lower leg muscles to the toes, enabling you to straighten and lift your toes.
• Flexor Tendons: Located on the bottom (plantar) surface of the foot, these connect lower leg and foot muscles to the toes, allowing you to bend and stabilize your toes against the floor

The human foot and ankle contain 26 bones that work together with 33 joints and various muscles to provide support, balance, and mobility. They are divided into three main sections: the ankle, midfoot, and forefoot

1. The Ankle & Rearfoot (Hindfoot)
The ankle joint (talocrural joint) connects the foot to the lower leg and allows for upward and downward movement.
• Tibia: The shinbone; its lower end forms the inner bump of the ankle.
• Fibula: The smaller calf bone; its lower end forms the outer bump of the ankle.
• Talus: The main ankle bone that sits between the heel and the lower leg bones.
• Calcaneus: The heel bone; it is the largest bone in the foot and absorbs the impact of walking.

2. The Midfoot
The midfoot creates the arch of the foot and acts as a shock absorber. It is made up of five irregularly shaped tarsal bones:
• Navicular: A boat-shaped bone located just in front of the talus.
• Cuboid: A cube-shaped bone located on the outer edge of the foot, in front of the heel bone.
• Cuneiforms (3): The medial, intermediate, and lateral cuneiform bones sit in front of the navicular.

3. The Forefoot
The forefoot contains the long bones and the toes.
• Metatarsals (5): These long bones connect the midfoot to the toes, labelled 1 through 5 (from the big toe to the pinky toe).
• Phalanges (14): The toe bones. The big toe has two phalanges (proximal and distal), while the remaining toes each have three (proximal, middle, and distal).
• Sesamoids (2): Two tiny, round bones embedded in the tendons beneath the big toe joint

We are running a national survey to better understand the experiences, needs, and priorities of people living with hypermobile Ehlers-Danlos syndrome or Hypermobility Spectrum Disorder in Aotearoa New Zealand.

The survey is open to people aged 16+, living in NZ, who have or suspect they have a hypermobility condition.

A lump on the side of your foot can be caused by several common issues depending on its exact location, texture, and whether it causes pain. Common culprits include bunions (at the big toe base), tailor's bunions( at the pinky toe base), or ganglion cysts (soft, fluid-filled sacs).
To figure out what the bump might be, consider these details:
• Location: If it's at the base of your big toe, it is likely a bunion. If it's on the outside edge at the base of your little toe, it is known as a Tailor’s Bunion (Bunionette). If it is near the ankle joint, top, or side, it could be a Ganglion Cyst.
• Texture & Pain: Soft, movable bumps that occasionally change in size are often cysts or lipomas (fatty deposits). Hard, bony bumps that ache or get irritated by shoes indicate bone spurs, bunions, or arthritis.

When to seek care:
While many bumps are harmless and can be managed with wider footwear or protective pads, it is highly recommended to see a podiatrist for an accurate diagnosis. You should book an appointment if the lump is painful, growing rapidly, red, swollen, or if you have diabetes—as foot complications need to be monitored closely

Your child's foot pain is often caused by normal developmental changes, active lifestyles, or something easily treated at home. Common culprits include growth spurts, overuse from running, or minor skin and nail irritations. However, persistent discomfort should be monitored.

The exact cause of the pain usually depends on where it hurts and your child's age:
• Heel Pain (Ages 8–14): The most common culprit here is Sever's Disease (calcaneal apophysitis). This is a harmless inflammation of the heel's growth plate, typically triggered by sports, running, or a sudden growth spurt.
• Pain at Night (Ages 3–12): This is often classified as classic "growing pains," which cause a deep, throbbing ache in the evenings or wake them up at night.
• Toe Pain: This is frequently caused by external irritations, such as an ingrown toenail, a blister, or plantar warts on the bottom of the foot.
• Arch/General Foot Fatigue: Flexible flat feet are very common in toddlers and young children. While rarely painful on its own, it can cause arches to ache or lead to foot fatigue after heavy activity

Hypermobile feet occur when the connective tissues and ligaments in the foot and ankle are overly lax. Common symptoms include chronic ankle instability, collapsed arches (flat feet), heel pain, muscle fatigue, and a high risk of strains. This instability frequently causes pain to radiate upward into the knees, hips, and back.

Key Symptoms to Watch For
- Ankle Instability: Frequent ankle rolling or recurrent sprains due to loose ligaments that fail to secure the joint properly.
- Collapsed Arches: Flat feet are highly common as gravity pulls the unsupported bones downward during weight-bearing activities.
- Heel Pain & Plantar Fasciitis: The muscles and ligaments overwork to compensate for loose joints, leading to intense heel and arch pain, especially after long periods of standing or resting
- Muscle Fatigue & Aches: Muscles in the feet and calves tire out quickly because they are constantly working to stabilize the joints.
- Balance & Coordination Issues: Weakened proprioception (spatial awareness) often results in clumsiness, stumbling, or a history of frequent falls.

I have some exciting news to share — I am now accepting referrals for the assessment of patients with suspected Ehlers-Danlos syndrome (EDS), including hypermobile EDS (hEDS).

Assessments will include a comprehensive clinical history and musculoskeletal examination, with consideration of relevant diagnostic criteria and differential diagnoses. Where appropriate, I can also provide recommendations for further investigation or specialist referral.
