🔹 Factors that Interfere with HbA1c Measurement

Genetic variants (e.g. HbS trait, HbC trait, HbE trait), elevated fetal hemoglobin (HbF) and chemically modified derivatives of hemoglobin (e.g. carbamylated Hb in patients with renal failure) can affect the accuracy of HbA1c measurements. Table 1 (📌 image) contains information for most of the commonly used current HbA1c methods for the four most common Hb variants, elevated HbF and carbamylated Hb.
When selecting an assay method, laboratories should take into consideration characteristics of the patient population served, (e.g. high prevalence of hemoglobinopathies or renal failure).

▪ Condition that shortens erythrocyte survival or decreases mean erythrocyte age: homozygous Hb variants, recovery from acute blood loss, hemolytic anemia, …

𝟐.𝟐𝐚. The A1C test should be performed using a method that is certified by the National Glycohemoglobin Standardization Program (NGSP) as traceable to the Diabetes Control and Complications Trial (DCCT) reference assay. 𝐁

𝟐.𝟐𝐛. Point-of-care A1C testing for diabetes screening and diagnosis should be restricted to U.S. Food and Drug Administration–approved devices at Clinical Laboratory Improvement Amendments (CLIA)–certified laboratories that perform testing of moderate complexity or higher by trained personnel. 𝐁

𝟐.𝟑. Marked discordance between A1C and repeat blood glucose values should raise the possibility of a problem or interference with either test. 𝐁

𝟐.𝟒. In conditions associated with an altered relationship between A1C and glycemia, such as some hemoglobin variants, pregnancy (second and third trimesters and the postpartum period), glucose-6-phosphate dehydrogenase deficiency, HIV, hemodialysis, recent blood loss or transfusion, or erythropoietin therapy, plasma glucose criteria should be used to diagnose diabetes. On December 7, 2023, Stago organized an online webinar about PT/INR coagulation test with sharing from Prof. Tripodi, Humanitas University & Angelo Bianchi Bonomi Hemophilia and Thrombosis Center, Milan, Italy

Prof. Tripodi clearly indicated the factors and their influence on the quality of PT results, including patient samples with pathology (Lupus, ...), plasma quality (with containing DOACs,..), the measuring system and the source of thromboplastins.

Besides the common application of PT/INR in anticoagulation monitoring, there are other crucial applications of PT in clinical practices:
🔸 PT in a combination of tests to monitor and diagnose disseminated intravascular coagulation syndrome (DIC), in addition to platelet count, Fib, FRMs tests (FDP, FM, D-Dimer).
🔸 Plasma DOACs concentrations can be preliminarily assessed if PT is abnormally prolonged. For samples containing DOACs, it is recommended to remove DOACs before performing PT testing.

📌Conclusion: Prof. Tripodi affirmed that PT continues playing an important and indispensable role in medical laboratories globally. PT assists in monitoring patients with congenital or acquired blood clotting abnormalities. PT is a prognostic indicator for patients with cirrhosis, as well as a solution to monitor the use of coumarin and vitamin K antagonists, …
