A company staff member may have started their business document from an online template, or a document used previously for an American client. In this case, they failed to realize that the template used American instead of Canadian English. In a busy environment where people are swampled with paperwork and/or computer data entry, the squad check is often dreaded. At some point we've all pinched a finger when closing a drawer or moving an object. It wasn't a fluke. The reason for hand injuries is lack of attention or awareness of our position.
