Hybrid start to finish!

James breaking down that smile and full-mouth cases get planned with all the focus on teeth first. They should be planned from the cranium. A tomogram shows you what the teeth can’t. As the dentition changes, the condyle shifts, the disc can displace, and the surrounding tissues get compressed or stretched. That’s where TMD symptoms start, and they look different in every patient.
The goal with a compressed joint is decompression: roughly 3–4mm between the condyle and the glenoid fossa, so the disc can move the way it was designed to.
Treat the joint first. Then build the smile.
