Did you know a system can pass installation, get fully signed off, and still not be ready for operation in any meaningful sense?
It happens more often than people are willing to admit, because once the paperwork is complete and the checks are done, there is a natural tendency to assume the job is finished, even when the system has not been pushed hard enough to reveal how it will behave under real operating conditions.
The truth is that many systems are only tested just enough to move forward, not enough to expose weaknesses, and that difference is where problems begin to take shape. When pressure is introduced and flow stabilises over time, small issues that were easy to miss during testing start to show up in ways that affect performance, reliability, and ultimately cost.
What makes this uncomfortable is that nothing appears wrong at the beginning, which makes it easy to believe the system was properly validated, when in reality it was simply not challenged thoroughly enough to prove that it would hold up in operation.
